| Amyotrophic Lateral Sclerosis (ALS) | (n, name) โรคกล้ามเนื้ออ่อนแรง เป็นโรคที่เกิดจากการเสื่อมสลายของเซลล์ประสาทที่ควบคุมการทำงานของกล้ามเนื้อ พยาธิสภาพของโรคเอแอลเอสอยู่ที่บริเวณแกนสมอง และไขสันหลัง ทำให้ผู้ป่วยมีกล้ามเนื้อแขนขา และลำตัวอ่อนแรง กลืนลำบาก และพูดไม่ชัด คำว่า “Amyotrophic” หมายถึง กล้ามเนื้ออ่อนแรง ฝ่อลีบและเต้นพลิ้ว (fasciculation) ส่วนคำว่า “Lateral sclerosis” หมายถึงรอยโรคในทางเดินของกระแสประสาทซึ่งควบคุมการทำงานของกล้ามเนื้อในไขสันหลัง โรคเอแอลเอสพบน้อยมาก โดยพบได้เพียง 4 ถึง 6 ต่อประชากร 100, 000 คน และโอกาสที่จะพบผู้ป่วยรายใหม่มีเพียง 1.5 ถึง 2.5 ต่อประชากร 100, 000 คนต่อปี โรคนี้เป็นกับผู้ใหญ่วัยใดก็ได้ แต่จะพบบ่อยขึ้นตามอายุ อายุเฉลี่ยของผู้ป่วยโรคนี้ประมาณ 65 ปี โดยมักเป็นในเพศชายมากกว่าเพศหญิง จนถึงปัจจุบัน ยังไม่ทราบแน่ชัดว่าโรคเอแอลเอสเกิดขึ้นได้อย่างไร ผู้ป่วยราวร้อยละ 5 (โดยเฉพาะผู้ป่วยอายุน้อย) อาจมีประวัติโรคนี้ในครอบครัว สิ่งแวดล้อมหรือสารพิษบางชนิด อาจมีผลกับการเกิดโรคนี้ เนื่องจากพบว่าในบางพื้นที่ของโลก เช่นที่เกาะกวม พบโรคนี้มากกว่าที่อื่น นอกจากนั้นการศึกษาทางระบาดวิทยายังพบว่า นักกีฬาบางประเภท โดยเฉพาะนักฟุตบอลอาชีพมีโอกาสเป็นโรคนี้สูงกว่าประชากรทั่วไป ปัจจุบันเชื่อว่าสาเหตุของโรคนี้อาจเกิดจาก กลไกภูมิคุ้มกันต่อตนเองผิดปกติ (Autoimmune attack) หรือเกิดจากกลไกอนุมูลอิสระ (Free radicals) ทำลายเซลล์ประสาทของตนเอง ส่วนอีกทฤษฎีหนึ่งเชื่อว่า การเสื่อมสลายของเซลล์ประสาทในสมองและไขสันหลังเกิดจากสารสื่อนำกระแสประสาท (neurotransmitter) ที่เรียกว่ากลูตาเมต (glutamate) กระตุ้นให้เกิดการทำลายเซลล์ (ข้อมูลจาก ผศ.นพ.รุ่งนิรันดร์ ประดิษฐสุวรรณ สาขาวิชาอายุรศาสตร์ปัจฉิมวัย : http://www.thonburihospital.com/th/tip_detail.asp?id=22 , http://www.inf.ku.ac.th/article/diag/520506/alsmnd.html) |

Cerebro-spinal sclerosis (Med.), an affection in which patches of hardening, produced by increase of the neuroglia and atrophy of the true nerve tissue, are found scattered throughout the brain and spinal cord.
